HART is a partnership of agencies and individuals working together to share information and resources to help the homeless in our area. As a Continuum of Care (CoC), we coordinate housing and services for homeless families and individuals and refer those in need to an appropriate program, if available, in their area.

CoCs are tasked to track and manage the homeless community in their area. One of most important activities entrusted to CoCs is the biannual count of the homeless population and an annual enumeration of emergency systems, transitional housing units, and beds that make up the homeless assistance systems. These counts provide an overview of the state of homelessness in a CoC, and offer the information necessary to redirect services, funding, and resources as necessary. The CoC also manages these services, offering both prevention strategies and homeless assistance programs to assist those at-risk of or experiencing homelessness.

Each year the CoC provides an opportunity to organizations in our service area to apply for funding through the Continuum of Care collaborative application process.

If you live or work in any of our 18-county Continuum of Care service area and have a desire to help, or are affiliated with any organization that offers services and support to the homeless, please consider joining us. We have monthly meetings that are normally held by Zoom on the second Thursday of the month at 10:00 am (central time). If you have any questions, please call our office at (931) 484-3207.
